How to make an appointment

Patient education is an important key to success in self-management of diabetes and other chronic disease. HEA accepts referrals from all practitioners in our community. To make an appointment, you may first talk to your practitioner and ask for a referral or call us directly at 707.443.0124 and we will request one. After we receive your referral we will contact you. You will be offered the next available class or an individual appointment as appropriate for your care.
