How do I access your services?
Call us at 707.443.0124, and we will get you scheduled. Referrals are required;
we can request one from your clinician for you.
When are your classes?
We have a new series starting once a month and periodic follow up classes in Eureka. Two class times are offered: morning and late afternoon.
Can I bring my family or others?
Yes, we encourage you to bring your family or other support people with you to class.
I don't speak English, are interpreters available?
No, we ask that you bring an interpreter.
What information is covered in the classes?
The class is taught by a registered dietician and a registered nurse, CDE. They
will teach you what you need to know to self-manage your diabetes. This
includes meal planning, counting carbs, the importance of exercise, medications
used in diabetes and assistance in making healthful behavior changes.
How much do they cost?
Each group session is $56.00 and individual appointments are $70.00 per hour.
Does my insurance cover the cost?
Medicare pays if you have met your deductible. Blue Cross HMO Plans pay. Blue Cross PPO and other private insurance plans may pay. Call your insurance group and ask if diabetes education is covered on your plan. If you don't have a secondary insurance there may be a small copayment due. We are not providers for MediCal. We offer a sliding scale, and most MediCal clients qualify for a $10.00 fee per class.
